98/hour Serve as a liaison between health and social services community to address the needs of children in foster care.
Assist foster parents in accessing medical, dental, and behavioral health services for children in their care.
Identify social determinants of health and provide care coordination and referrals to community resources.
Improve the lives of children in foster care through a range of activities including outreach, community education, informal counseling, social support, and advocacy.
This is a full-time, benefit eligible position working Monday-Friday 8:
00 am to 4:
30 pm.
ESSENTIAL FUNCTIONS - JOB SPECIFIC Support foster parents in coordinating and scheduling medical, dental, and behavioral health appointments for children in foster care Complete PRAPARE screening with target population to identify and address social determinants of health Provide case management including systematic and proactive follow-up Maintain working knowledge of Show Me Healthy Kids program Provide care coordination, which may include facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up Participate in community coalitions, groups, and meetings to gain better understanding of available resources.
Cultivate relationships with community partners to support cross-referrals Advance patient's health literacy Communicate with patients, caregivers, families, and care team to assist in meeting patient goals Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
